How do I enroll in Medicare Part B after age 65?

If you delayed Part B because you had coverage through current employment, forms CMS-40B and CMS-L564 can help you enroll during your Special Enrollment Period.

If you are past the seven-month enrollment window surrounding your 65th birthday and delayed Medicare Part B because you had health coverage through your or your spouse’s current employment, you may qualify for a Special Enrollment Period.

You will generally need two forms: CMS-40B, the application to enroll in Part B, and CMS-L564, which proves that you had qualifying employer coverage.

You complete Section A of the CMS-L564 and the employer completes Section B.

You can submit the forms online, by fax or mail, or take them to a Social Security office—and if you go in person, I would not go first thing in the morning because all the other old people will already be in line.

Clearly state the month you want Part B to begin, and do not wait too long after the employment or coverage ends because this Special Enrollment Period generally lasts eight months from whichever ends first.
